Full Job Description

Housing Advocacy Specialists work with homeless assistance and prevention programs. Responsibilities include:

Case Management: Assist families in identifying strategies to achieve realistic goals; serve as an advocate; make referrals to other programs; assist in skill development; participate in agency teams to provide comprehensive services; provide on-going case management/service coordination.

Community Outreach: Maintain partnerships with community organizations, volunteer networks and the greater community; reach underserved populations for agency programs.

Program/Agency Compliance: maintain working knowledge of project grants; collect and submit required documentation; comply with Outcome Measures tracking and reporting process; data entry; attend department and agency required meetings and trainings.

This full-time position (40 hours per week) will be based in Zumbrota. Occasional travel using a personal vehicle is required; mileage reimbursement is based on the IRS mileage rate (currently $0.655/mile).

Qualifications:

  • Bachelor s Degree
  • Licensed Social Worker (preferred)
  • Previous case management/service coordination experience
  • Computer and data entry proficiency
  • Ability to maintain confidentiality and follow data privacy policies and procedures

Functional Requirements:

  • This primarily office-based position requires lifting 20 pounds occasionally; communicating verbally in person and by phone; and seeing documents and computer screens
  • Ability to travel using personal vehicle to meet with clients
