Job Overview::The purpose of this position is to plan, direct and, supervise duties relating to the overall corporate Supply Chain program. The Supply Chain program spans all purchasing and inventory control for the Consortium to ensure that the Consortium purchases and receives supplies and services in a fiscally efficient manner. In collaboration with the Office of the General Counsel, internal customers and vendors, facilitates the Consortium’s competitive bidding process (“RFP”) as each project dictates. This position serves as a key relationship manager for the Consortium’s Group Purchasing Organization (“GPO”) partner(s) and selected vendors, leads and/or coordinates the Value Analysis process and the Value Analysis Committee (“VAC”) for the Consortium. The position will collaborate with the Information Systems Department and other key stakeholders to maintain the integrity, financial controls and efficient use of all purchasing & inventory management ERP and stand-alone technologies. The position maintains, and regularly reviews and/or updates all Supply Chain-related policies and procedures to balance efficient procurement with effective audit controls; and in collaboration with general counsel and accounting, communicates the policies Consortium-Wide. Other functions include serving as program administrator for the Commercial Bank Purchasing Card Program.
In addition to performing line functions, the incumbent provides staff consultation and advisory services to field operations.
Responsibilities::

Manages and monitors assigned client caseload using a variety of interpersonal skills that support age-appropriate independence, autonomy, safety, and stability of the identified child(ren).
*

  • Provides skill development for the young person and their family/care providers using the teaching family model to build social, cognitive, and living skills. This includes assisting young people and families to identify and secure needed resources to meet their treatment goals.
  • Facilitation of level-of-care transitions.
  • Assists clients to link and coordinate with health and social service support, e.g., medical, hospital, psychiatric, substance use, housing, food, clothing, financial assistance, education, employment, legal, social security, tribal organizations, and other community/social services.
  • Provides supportive active listening to engage, encourage, and motivate clients toward age-appropriate self-directed support and change, while assisting care providers to understand development needs and capacities of children and youth
  • Documents all services and encounters in compliance with organizational standards and timelines. Completes necessary processes and documentation for prior authorizations required to obtain additional testing, services, treatment, or medication as part of the treatment plan.
  • Promptly responds to urgent client circumstances for assigned caseload and others when directed by supervisor or treatment team leader.
  • Works closely with Referral Care Coordinators, and when necessary, processes referrals to effectively facilitate internal and external care referrals.
  • Proactively identifies delays and obstacles to care and works to remedy individual cases and improve processes.
  • Provides outreach and advocacy in the community to meet the needs of the client and their family.
  • Facilitates individual and group psycho-education skills and classes as described in the treatment plan.
  • Attends and participates in staff meetings and mandatory trainings. Maintains skills, annual competencies, and certifications, and participates in in-services and educational programs as assigned.
  • Interacts with clients, peers, colleagues, and internal and external customers while upholding the Mission, Vision, and Values.
  • Completes and maintains patient specific training that is reflective of the staff’s caseload. e.g. Substance, Crisis, or other therapeutic skills and techniques.

Qualifications::*

Education:**
Prefer- BS/BA in social services, education, human development, mental health, or related field. An equivalent combination of education and experience can be exchanged on a year for year basis with the BS/BA requirement.

  • Prior experience in general case management, social work, or counseling, preferred
  • A minimum of 6 months addiction services experience preferred
