Care Manager - East Region
Job Description
The Care Manager provides comprehensive care management to ensure integrated, whole person care for the individuals we serve. The Care Manager must be highly organized and possess excellent leadership skills. This position interfaces with many local and state-wide partners and requires the ability to develop and maintain positive relationships with a number of Tailored Plans and professional disciplines, such as psychiatry, psychology, medicine, social work, and occupational therapy, speech therapy, physical therapy, teaching, and direct support professionals for the benefit of the individuals being supported. The Care Manager must use professional judgment in conducting in-depth individual assessments and providing supportive counseling and guidance. The Care Manager will serve as the primary advocate for the person within the service system(s) and the community.
Reports To: This position will be under the direct supervision of the Senior Care Manager

FLSA Status: Full-time, Exempt
Salary Range: $45k - $52K
Minimum Requirements:
· Care Managers serving members with I/DD or TBI must qualify as a Qualified Professional (QP) in North Carolina and have one of the following minimum qualifications:
· A bachelor’s degree in a field related to health, psychology, sociology, social work, nursing, or another relevant human services area, or licensure as a registered nurse (RN); and
· Two years of experience working directly with individuals with IDD, TBI and/or dual diagnoses of IDD and MI; and
· For care managers serving members with long-term support needs: two years of prior long-term support and/or Home and Community-based Services coordination, care delivery monitoring, and care management experience, in addition to the requirements cited above. (This experience may be concurrent with the two years of experience working directly with individuals with IDD, TBI, or dual diagnoses outlined above.)
Must possess a strong working knowledge of the service delivery system and funding streams, good communication skills - verbal and written and proficient with computers and EHR. Must have a valid NC driver’s license and current automobile liability insurance.

Pay: $45,000.00 - $52,000.00 per year
